Not receiving notification of comments to be moderated

Topic title is self-explanatory.

I’ve found much discussion here around solving the problem where no notifications at all are getting through, but we are receiving notification of comments that have been approved, just not when modification is needed.

Has anyone else encountered this, and if so, were you able to fix it?
