Where is the Thank you Page?

First of all – thank you for this Plugin, I know it is a lot of work and I really appreciate all the hard work that is going into this plugin.
I installed it and have it setup, seems to be working BUT for some reason when I use the data-request form I get an error page from my blog after the sending. It seems it is unable to display the Thank you page.
Where and how would I define it?

The Update broke the plugin.
Now not even the text is shown when you want to comment ??
See this screenshot – it simply has a box now with ARRAY next to it
