Users show up as ‘Anonymous’ on results under quiz

Before taking the quiz, there is information captured using the forms on the quiz options.
After a user takes a quiz, the auto generated email will say anonymous user where the user name token is entered.
When you click results under the quiz every quiz taken is labeled anonymous until you view the details then the captured information is then listed.
How can we get the auto generated email to say the users name and also the list of results, how do we get the list to show the users name?

Any help would be appreciated!
