The “Add Slide” button is missing

When creating a slideshow I am NOT able to add more slides to the existing slideshow because the “Add Slide” button is missing at the top right of the plugin’s admin interface.

IMPORTANT NOTE:
The “Add Slide” is button is initially there when making a new slide, BUT the button disappears AFTER adding some slides so that NEWER slides CANNOT be added to the existing slideshow.
