Is the submission form only used by logged-in users of your website?

If yes:

1. Enable the “Require login” option
2. Edit the shortcode to hide the name and email fields

i.e. [site_reviews_form hide=name,email]

Submitted reviews will now automatically use the name and email of the logged-in user.
