Error message exclusive checkboxes or radio buttons

  • I set up a form and tried to use exclusive checkboxes or as an alternative radio buttons (see first item contact form).

    But after clicking the submit button there’s an general error message that appears right below these checkboxes.

    After trying many things I don’t know how to get this form working properly. There seems to be a problem when I use exclusive checkboxes or radio buttons, but I don’t know how to solve this.
