Problems with bundled items

When I use the Phone Orders plugin to create new orders containing bundled items there are incorrect duplicates of each bundle child item also added to the order. If I attempt to remove an incorrectly duplicated child item from within the phone orders page the item is re-added to the order immediately after the removed item is dropped from the order.
The first instance of the child bundle items shown in the cart are priced according to the bundle configuration, but the duplicate bundle items are shown with $0 price and $0 subtotal.
When the phone order is created it also adds flat-rate shipping for the physical bundle child items (including the incorrectly duplicated bundle child items) and the bundle parent items. These same items added through the customer shopping cart only add flat-rate shipping fees for the bundle’s physical child items.
How do I get the phone order plugin to stop incorrectly adding duplicate bundle child items and also stop adding incorrect flat-rate shipping fees for items that should have no shipping charges?
I’m using the WooCommerce Product Bundles plugin to bundle a physical child item with a virtual child item.
