Plugin still supported?

Hi there,
I am trying to test this plugin for a client for a subscription-based premium content site. I’ve signed up for a try account with Chargify, created a couple test products in Chargify, generated the API keys, installed this plugin and created the pages on the test WP site as directed in the install docs.

However, my subscribe page doesn’t seem to “go anywhere” when I fill out the form and click the button on my subscribe page. The buttons are not actually linked to any of the Charigy signup pages…?

I’ve verified that the API is saved in my Chargify Settings screen, as well as enabled and synced the products in the Chargify Product Settings page.

Help! What am I missing?
