Groups not showing

Hello. I have the same problem with “Contact Form 7 Conditional Fields” on the following page
https://ciaps.org/online-registration-form/
In the above form, the field “How did you hear about CIAPS?” is supposed to work like it does on the page
https://imageazy.com/contact/
but the hidden fields remain hidden.
I have tried switching theme to Twenty Sixteen and turning off other plugins (in the middle of the night) to troubleshoot it but can’t find a solution. I must also mention I’m not so php savvy.
Please help me take a look at it.
