Opt Out Choices not showing

Create an account as subscriber or any account type and the front end or back end notification opt in and opt out form as not showing up. If a user is already opt into a notification the choice will show up to opt in and out of a notification. If the notification exists, but the account type is not signed up for it the option won’t even show up.

URL (after signing up): https://stg.alamedactc.org/manage-subscriptions/ is blank if a user is not already opt into notifications type. I have about 10 different notification types, but want subscribers/other account types to pick what they want to opt in and out of. Do I have to opt all users to these notifications first and then have them decide? The form is blank if a user is not already signed up for a notification by an admin in the backend. Defeating the purpose of opting in and out. Please help, I have the add-on and everything.
