Form only showing to logged in users

    I’m having an issue with my form which is causing the form not to load to users who aren’t logged in. For some reason there’s code being wrapped round the form automatically 

     Although when I remove this code it still doesn't show the form. The shortcode of the form is [CP_CALCULATED_FIELDS id="7"] 

The settings I have selected are script load method : classic
Character encoding: keep current charset 
activate javascript caching 
If there's anything I've done blatantly wrong or any troubleshoots please let me know, this will be much appreciated. 
My thought is a plugin conflict.
