Dates

I’m trying to import a date from a Gravity Form into Salesforce using the CRMPerks plugin. The Salesforce API wants the date in YYYY-MM-DD format. What is the process to set that up in the plugin? I see three separate fields Date (YYYY), Date (MM), and Date (DD) in the plugin when I attempt to map them. My efforts at creating a custom formula have not worked.
