MY MISTAKE-Form file upload suddenly not working

    The file upload is suddenly not working on this form. It has worked dozens of times before and just stopped. When you click the button to upload the file, it doesn’t appear to upload anything and then because the file attachment is a required field, it won’t let you submit the form because there is no file. help??
