How to to collect the URL?

    Hi there,

    I bought the upgraded version. Very nice!

    However, I a a few questions:

    1. I have to collect the URL of the page who contents the form. How can I add the referral?

    2. Is it possible to complete automaticaly a Google sheets with the results?

    3. The form is on a sidebar (see capture). How to keep the form fixed (always visible), even if the user scroll?
