    I’ve visited your site and the form shows up from my end. Please see this screenshot:

    Would you please flush site’s and server’s cache and see if the issue will be resolved.

    If not, please let us know is the issue happening from ll kind of browser from your end. Let us know which one and their version, and your OS system.

    Oh yes it’s up. I turned off ajax and it worked. Just forgot to mark this as rresolved.
