All form fields not showing in notification email

Hi,

I recently installed your Plugin. Everything is working fine except for the Email Notification.

As you can see, all the form fields are not visible in the Email. Even some of the fields marked as “required” (such as “full name”) are not shown along with some fields which have been filled while filling up the form.

I also need to change the subject of the Email Notification that I receive.
