[Plugin: Contact Form 7] Footer Disappears when inserting code

I’ve just copied and pasted the necessary tag into the page I want the form to appear (https://www.wakatapware.co.nz/cad-files/) and it doesn’t show the form nor the footer.
Where am I going wrong?
