Run only on specific page / only for logged in users

Anyone know of a way to make this plugin execute only on a specific page? OR alternatively, a way to make it only run for logged in users?
It doesn’t seem to play nicely with my caching plugin, which is to be expected. So I’m looking to either limit it to the one page I need it on (where caching is disabled) – or to logged in users (where caching is disabled).
Any help in accomplishing either of these methods would be greatly appreciated.
