by the way, thanks for fixing the “Does not work in WordPress 5.5” issue

unless i’m a dummy, and there are those that may posit this, there seems to be a lack of CSS selectors for styling elements of the form – for instance, the “Categories” text

also i might suggest renaming the .checkbox class to something less generic – .stc-checkbox

lastly, i wonder if you might consider dumping the “E-mail Address:” label in favor of a placeholder for the email input – something like “Your email address …”
