Site Survey / Questionnaire – login and relogin to continue entry

I’m like to have clients, login into a wordpress ‘page’ and fill out a questionnaire.

I’d like it some they can come back at any time – and continue entering data.

My preferred format is multi choice questions with either radio buttons that allow mutiple selection for questions

eg Which Email platforms do you currently use O-Lotus ; O-Exchange …

and they can then ‘tick/select, the answers.

Potentially also free form text for each question so they can enter further details.

The key is
1) The login using a password created for them
2) The clients/responder’s ability to to complete the questions over a period of time – with content already entered saved
3) The data is email to me or I receive a flag indicating the form has been completed.

A few thoughts:

Contact 7 – looks like a candidate?? – Not sure about the login/logout and returning to the form later for further data entry

Would appreciate any thoughts/experience etc in this area
