Getting Some Email Notifications but Not All

Over the last few weeks I have had 10+ clients contact us and say that they submitted a form even though we never received notification that it was submitted. We are receiving email notifications for some of them but obviously not all of them. Any idea what would cause us to get notification for some submissions but not others? I absolutely need to know when the form is submitted. I am using a @therouteexchange.com addresses for the From: field in the mail settings….
