Packing Slip fields

Hello,
we need to show some fields we added on the checkout into the packing slip.
This fields we need:
– Name
– Lastname
– Date of birth
– Order number
– Serial key (that is generated by other plugin, this already shows in the packing slip)
– Affiliate or Coupon usedand also we need to add some static text.
How can we accomplish this?
