I use once a month for database optimisation only. Quick and easy – remove post revisions then optimise the database. Be aware that optimising an INNODB database works but with the default MySQL configuration actually grows the database file size as optimised tables are appended. Changing this is described in Howto: Clean a mysql InnoDB storage engine? under the section “InnoDB Infrastructure Cleanup” and works beautifully. Optmised database files are then also smaller than before.
