Select Field is Missing in Email

The same is happening with my forms.
Date, and other select fields are not shown in Entry or Notification Email.

It seems the name you have used for the select field is incorrect and that is why the select fields value is not coming. Please check this screenshot for clarification.

Please follow this rules for a input field – Enter a name without space or any special character. Use only underscore/hyphen (_/-) for multiple words. The name must be unique.

Can you please follow the above rules I have mentioned for the form field’s input name? Hope that is also the solution of your problem as well.
