Double products in product order

Hopefully you can help me with this problem. Sometimes whenever an order is placed, there will be double products on the receipt. It’s a very strange error because the customer sees the double products at the check-out, but the subtotal is equal to the “normal” products they ordered.
For example: a customer wants to buy two products. The subtotal in the cart is worth these two products. When the customer checks-out, he sees that there has been added 3x from the same products to the receipt. There is a bug somewhere, but I can’t seem to find it.
This problem doesn’t always appear, sometimes it just comes out of nowhere and there’s no pattern to find.
