Personalization in forms

Hi All, would someone please suggest a good plugin to create a form with the ability to use personalization in the form?

If someone fills their name in step 1 then I can use it in step 2. (ex. Thanks for the info {first_name}, now please complete this section) or (I see you like {blue}, here are some blue jeans in your {size}.)

Then, my client then also wants to be able to offer specific products at the end based on the information supplied by the end-user. So was thinking of conditional logic but wanted to ask if there is another way.
