checkbox at top of form

    So I am getting a check box at top of the form. If I check one of the boxes it also checks the one at top. How to remove this extra box?

    I do have a license but since this is for a dev site till it is ready cant use it.

    On checking the form URL, I see that the extra check box issue has been resolved. Could you please confirm?

    If you have an active license subscription with us, could you please submit a support ticket through the WPForms account dashboard when you have a chance?
