Could not process order. Order status changed from Pending payment to Failed.

Hi,
We seem to have a problem with failed payments, it seems like they fail before they get to Paypal because we end up with failed orders that have this status and no order reference.
Could not process order. Order status changed from Pending payment to Failed.
On the orders page at the top they say
Payment via PayPal. Customer IP: X.X.X.X
So its like they fail before they get an order reference from paypal.The logs don’t seem to show anything useful.
We are on Version 1.9.1I can send you log files if that would help.
