Adding Specific Comment Form To Specific Page

Hi

So I have created 2 comment forms. One is general for posts comments and the other is a specific form for specific roles to do a Q&A.

I use the comment excluder plugin. I have it set to allow comments on both posts and pages.

On the specific page I checked discussion / allow comments.

The regular post comment form shows up on the post pages and page page fine.

For the QA form I set the page ID (1681) in that field, I unchecked post and checked page since it is a page, but it doesn’t show up.

Is there no shortcode to just add the form where we want it? Or the comment form ID?

All the posting permissions are correctly set for wordpress, the exclusion plugin is not blocking it, so the culprit has to be wpdiscuz, which usually means it’s actually something I did wrong. lol

Any suggestion as to what I might be doing wrong?
