    Hi There!

    Recently some of the forms I created in forminator suddenly become unusable, the form (inputs & labels) went completely hidden on the page, remain only empty button.

    Meanwhile, others form is working perfectly fine. I had turn on troubleshoot mode which only turning on Forminator plugins while the rest of the plugins is deactivated, but the problem is still remaining even I changed the theme to Twenty Twenty-One.

    I am not sure where the bugs/errors, not sure how to fix this problem.

    Hi @nightck

    I hope you are doing well today.

    Based on your page source, it seems that the form generates correctly and it is fully empty like no fields are added. There are also no errors in the browser console.

    Do you see any kind of fields in your form EDIT MODE? If so, can you export your form, upload it google drive and share a link in your next reply so that we could review it on our lab?

    Sorry for the late reply,

    In the ‘EDIT MODE’, all the fields that I created for the form have all went missing as well as the title asking me to enter a valid name.

    I also export the bug form, just to test out if can spot any error from the export.

    Hi @nightck,

    Was the website recently migrated? Do you recall any recent changes done on your website that might have affected the database?

    The form export you have shared is also empty. Is the issue only specific to one forms?

    Do other forms load fine? I could also notice you are running an older version of Forminator ie v 1.20.1.

    Could you also check and see whether updating the plugin to the latest version makes any difference?

    If you still notice the same issue then please disable all the plugins except Forminator and switch to a default WP theme and see whether you get the same issue or not.

    Yes, the website was migrated not long ago. I can’t recall of any changes was done that would affected the database but mostly likely something happened here.

    I have like 17 active forms but 7 out of 17 was like that empty forms, when opened which gave me empty forms.

    I had updated the plugin to the latest version but doesn’t seem to fix the problem.

    Another method I had try was using FileZilla to upload old backup of the Forminator data/folder into the live server, but nothing changes or solve the problem.

    ^^ above image is activated troubleshooting mode with only Forminator plugin and the rest of the plugin was disabled.

    with the theme of twenty twenty-one, the form is still empty.

    Hi @nightck,

    Thanks for the follow-up. Unfortunately, the issue still looks more specific to the server side with the missing database.

    There isn’t any setting on the plugin side that would cause the form to get empty. Since the issue is still the same even after following all the mentioned steps, I’m afraid the easiest workaround would be to revert the full site to a working backup.

    Maybe it’s something you could bring into your Hosting team’s attention too.

    Please do let us know what your hosting support has to say regarding the issues noticed.

    Hi Nithin

    Thanks for the prompt reply. Apparently the issue was on the server side. My colleague was able to find the bug as after migrating to a new domain/server, somehow the database was missing.

    Since the bug was caused by a missing database after migration, we are performing a full revert with a working backup right now.
