Pass through custom field and rename file

Hi,

Can I pass through a custom field ie, page_id and create a new upload folder for that file to be uploaded to, and also rename the file.

ideally if I could upload multiple files to custom upload folder and give them auto incrementing filenames would be perfect.

Happy to buy the pro version if this is conceivable and or pay somebody willing to code the necessary changes to make if work.
