Shipping Option Not Showing

I have had this plugin installed and working on my site for sometime. However now it has recently stopped working. It is still setup and turned on as a shipping option for both shipping zones. I haven’t changed anything on the shipping settings but it is no longer showing up on the cart/checkout page.

Any ideas why it would suddenly stop working?
