Extra field displayed (Gravity Forms)

Hello, when we activate CleanTalk, we are seeing an extra field on all forms.

Some forms have a hidden field, but also see the extra field on forms without hidden fields. I’ve done the full deactivation and tried re-installing the plugin, but they still display when logged out. When logged in, they are not visible. Here’s what I get when inspecting:
