[Plugin: G-Lock Double Opt-in Manager] Plugin Doesnt shows Registration Form

The plugin doesn’t shows the registration form. On the backend, even though I click on ‘Default’ button, I get nothing.
I even removed all the plugin options from the DB (using clean options plugin), but nothing works.Any tips/tricks?
