Same items in cart breaks installation

We discovered that if we add a product to the cart WITH installation, then add the same product WITHOUT installation, the installation fee is removed and can no longer be added – even after clearing the cart and starting over.
It used to show 2 line items, and have the installation cost for the correct line item, so something has changed.
For settings, The fee is ‘Multiply Cost-Yes’ and shown as a global fee.
