Here is an example, The questions are in three-column format in step 3 but they are in a list when filling out the form.

Hey @scottaarondunn – I looked into the form, and it seems that you are using the paid version of WPForms. If you have an active license subscription with us, could you please submit a support ticket through the WPForms account dashboard when you have a chance??
