This seems to be related with one of your plugins or active theme conflicting. Therefore, I suggest to perform full plugin/theme conflict test.

Please make sure you have recent backups and perform this operation on your dev/staging site if possible.

If the issue persists, please share an export of the form with us and Site Health report which you can get from the WordPress admin dashboard Tools->Site Health->Info->Copy site info to clipboard, so that we can check further.
