Missing Default form – HELP!

  • Hi All,

    I just bought Points and Rewards Pro and it seems like i do not have a default WP form on my page (which they confirm it will only work with that) They do not work with other plugins.

    I think I may have deleted it – I cannot remember.

    How Can I get this back? Thanks in advance.
