I noticed that in my DB wp_w3tc_cdn_queue table I have 11 errors saying “empty host”. Not sure how to interpret this error. I don’t have CDN enabled.

Sample path in the DB (with this error):
    /home/domain/public_html/wp-content/w3tc/min/abf311/attachment.include.1366857189.js

    Advice is appreciated.

Did you ever turn on the CDN at any point?

Not intentionally, but I think at some point I noticed that checking the checkbox in the general settings page, saying “deselect this if you want all caching disabled” actually turned on some features (CDN included).

For the moment I had to disable / deactivate W3TC because of another error: 3.0.3 – blank page after publishing, so I can’t check any option in the dashboard.

I would really really love to see the plugin working with 3.0.3. as it is one of the “must-haves”.

Did you submit a bug submission form from the support of the plugin about the issue?

No, I didn’t. But I will do so the next time I see the error.
