Could you please share a screenshot of the form in your page editor. Since you can’t attach images here directly, you can send it via Google Drive, Dropbox, or a similar file/image sharing service.

Please also let us know which page editor you are using, and where exactly do you paste the form’s shortcode (for example, whether you insert it in some block, etc.).

I am using a plugin called Spectra, and all the blocks are from this plugin. I inserted a form (the default block from Forminator) into a Container block from Spectra.

I can see error in the browser console which says:

Blocked loading mixed active content and it is related to CSS file from Forminator.

Please visit WP Dashboard -> Settings -> General and check if “WordPress Address (URL)” and “Site Address (URL)” use HTTPS in the URL.
