I hope you are doing well.

I checked your website, but I can’t see any form being loaded, despite confirming the HTML shows the Forminator form should be loading in a specific area.

This issue and the one you described may be related to a plugin conflict.

Please create a Staging area https://www.ads-software.com/plugins/wp-staging/ and then run a full plugin & theme conflict test following this guide https://wpmudev.com/docs/getting-started/getting-support/#conflict-test
