facebook badge dissapeared

My website is toshmcintosh.com. The facebook badge in my 2010 Weaver sidebar disappeared a few days ago and the space showed only my name and below that “create a badge.” I’ve followed the instructions three times to fix the problem and nothing works. This is the text that is provided to copy and paste for the badge:
