[Plugin: Contact Form 7 to Database Extension] WordPress Database Error after upgrade

  • Plugin worked great until upgrade. After the plugin was reactivated, I get the following errors on every page (front end and admin) unless I deactivate the plugin:

    WordPress database error: [Duplicate key name ‘form_name_idx’]
    ALTER TABLE wp_CF7DBPlugin_SUBMITS ADD INDEX form_name_idx ( form_name )

    WordPress database error: [Duplicate key name ‘form_name_field_name_idx’]
    ALTER TABLE wp_CF7DBPlugin_SUBMITS ADD INDEX form_name_field_name_idx ( form_name, field_name )
