Form not working properly in IE 8 & IE 7

    This form will not work properly in IE 8 or IE 7.

    When the use clicks “I Wish To Evaluate,” the actual evaluation fields should appear below (using conditions). In those versions of IE8 or 7, they do not.

    It seems to work fine in IE 9, Firefox and Chrome.

    I really need assistance on this as a lot of our users are using IE 8 and are unable to access the entire form.
