    Due to the fact that I haven’t found any solution to this problem, I had to establish the form manually, without the use of this plugin.

    Would you be able to give me an idea how you created the form manually? I really need to do this myself – I need to restrict the answers to Yes or No responses.

    I got it from SalesForce backoffice. You got there an example for contact form. I believe you can look at the form that the plugin generates to you, and just copy the code and manipulate it.

    Dropdowns are not currently supported, but I’ll put it on the list for a future version.
