[Plugin: Events Manager] Customize Event Submission Form

I am trying to figure out how to allow subscribers to create their own events that are pending approval. I cannot figure out if I need a separate plugin for subscribers or if I am missing something in the documentation. I enabled the form for the beta version of anonymous submissions but I would prefer to have them be individual members/subscribers.

With that I also need to customize the form to ask for additional standard info, IE website, contact, etcetera. How do I revise these items?
