contact form with options

  • Dear WordPresser,

    I am looking for an contact form with which I can let people choose betwee several options.

    The idea is that someone should be able to choose betwee two options. If one of those options is chosen, a few new sub-options appear and so on…

    I am hoping someone knows an existing plugin to do this, I can’t find it in the plugin directory.
