How can I add an image upload form to my site?!?

I want to allow users of my site to upload a single photo (not from the dashboard). I have tried simply creating a form with a “file” field but the file never gets sent to the POST and REQUEST once the form is submitted! Someone please help, I have been looking for the answer to this now for a few days but came up with nothing, thanks.
