Forms for members

Hi all! I am looking for a way to have order forms in members profiles or in a post of some sort that links to particular members or something to that effect.

Basically, some of my members are suppliers of manufactured goods. I would like people to be able to fill out a materials list that is then sent to the supplier they wish to do business with. Is there a way anyone can think of to impliment such a thing? I would love if it could all take place on my site (messages sent and recieved via their profiles mail).

Any thoughts would be GREAT!
