Trackbacks Showing Blog Name Instead of Post Title

I dug around the forums and saw that this question was asked several times 4-7 years ago but most of the time wasn’t even addressed and even in the cases where it was, I never an actual *fix*.
I’ve only recently found the free time to be active on my company’s blog, but the first time I tested out trackbacks I ran in to a hiccup – When trackbacks are accepted, they are showing the main blog name (blog home page title tag) as the linked text, rather than the individual post name (or at least a combination like “blogname: post_title”). Not only does this look crappy in the comments section on these other blogs, but I suspect there are other bloggers who would see the trackback requests come in and ignore them on the suspicion they were spam.

Additional info: I’m using All in One SEO Pack, but I haven’t made any sort of strange settings changes. My post title tag setting is %post_title% | %blog_title% and they show up fine on the site. The only issue is how they’re showing in trackbacks.
